PROCEDURE FOR ADMISSION
1. How to obtain the application form and Entrance Examination fee payable.
2. Go to our website Online Admissions Registration Form Link and full fill form and Registration fee complete form Print and contact to office in working daye’s .
By Hand : Candidate may obtain the information brochure and application form by paying Rs. 100/- from the office of St. John Para Medical Institute.
By Post : Candidate may obtain the information brochure and application form through post by sending a crossed demand / bank draft for Rs.125/- drawn in favour of Director, St. John Para-Medical Institute, payable at Faizabad (U.P.). Candidate should write his/her name and address on the back of the demand / bank draft.
2 Candidate should fill the admission form in his/her own hand writing and submit in the office.
3. Students are selected purely on written test or interview basis will be taken by the Institute.
AGE & SEX
No restrictions with regard to the age of the candidates, both sexes can join this course.
INSTRUCTIONS FOR COMPLETING AND SENDING THE APPLICATION FORM
1. Do not use the photocopy of the application form
2. Candidate submit only one application form for one course.
3. Candidate must submit all the necessary document along with application form.
4. Necessary documents:-
(a) Five recent passport size photographs of the candidates are required in which three photographs firmly affixed in application form and two photographs attached with application form.
(b) Write the name of candidate on each photograph.
(c) Attested copies of mark sheet & certificate of High School, Intermediate must be attached along with application form.
(d) Character certificate from of the last institute/ college attached by the Candidate.
(e) Adhar card Photocopy
(e) If the Candidate belongs to any reserved category. Then he/her must enclosed the relevant certificate with application form.
5. Declaration by the Candidate:
The candidate must sign the declaration. The date and place should also be entered. The declaration by the candidate must be countersigned by parent / Guardian. Write the name of parent/ Guardian in the space provided.
